Student Setup - Immunization

The following data elements are required for student setup. The first two pages of navigation are removed for brevity:

Start Page > Student.

On the Start Page, select the School link at the top of the page.
Choose the appropriate school from the pop-up menu.
Click Submit.
Select the first letter of the student's last name.
Select a student from the list. The last accessed student information page opens for the student selected.

Vaccine	Click the vaccine name and choose an Exemption type from the pop-up menu, if applicable. Optionally, select the Out of compliance checkbox and/or enter a comment. A history of chicken pox is determined by assigning the Exemption "History of Disease / Parental Recall of Disease" to the student for their Varicella vaccine. Note: Vaccines and associated doses are set up at the district. For a list of valid vaccines, see the Appendix. Date each dose was given	Click the dose cell for the appropriate vaccine. Choose a date using the calendar icon, and then choose a Certificate Type from the pop-up menu. A certificate type must be chosen when entering a date; it is required for the page to function. A default certificate type is automatically set up with the Arizona state reporting installer. Additional certificate types can be set up at the District.
