Immunization Compliance

The Immunization Compliance report evaluates student immunization compliancy and reports exemption status based on the state's immunization rules for each child's age range. For more information on immunization rules, see the "Guide to Arizona Immunizations Required for School Entry: Grades K-12" document at the following website: https://www.azdhs.gov/preparedness/epidemiology-disease-control/immunization/index.php#schools-immunization-forms

Selection Criteria

Selection criteria determine which database records are used in the report. Refer to selection criteria for analysis when the report does not return the correct records.

Student Selection

The report selects records from the [Students] table based on the following criteria:

  • The student must be enrolled in the school on or between the School Start Date and Date of Report entered at report run time.

Note: The report uses the Date of Report to determine the student’s age for immunization compliance.

  • The student’s entry date does not equal his exit date (no shows or summer withdraws)

Additional Student Inclusion Note: Students with invalid vaccine dose dates (incorrectly formatted data) are not included in the compliance evaluation results. The incorrectly formatted data causes report instability, so the students with this data condition are not included in the evaluation. To include a list of students with invalid vaccine dose dates, check the appropriate box at report run time.

Report Input

For help with navigation and running the report, see How to Find and Generate a Report.

Field

Description

Current Selection Students

Indicate which students to include in the report by selecting one of the following options:

  • The selected [number] students only – Run the report for students in the current selection.
  • All students – Run the report for all students in the current school or district that meet the selection criteria.

Note: If running the report for a single student, or group of students, select the students prior to running the report.

School Start Date

Enter the first day of the school year.

Date of Report

Enter the date of the report. Immunization compliance is determined based on this date.

Include list of students with invalid vaccine dose dates

Check the box to include a list of students with invalid vaccine dose dates.

Students with invalid vaccine dose dates are not evaluated for vaccine compliance. The invalid dates will need to be corrected so that the affected students may be evaluated in a future report run.

The students with invalid dose dates are listed at the bottom of the report results. The list includes the vaccine name and dose number of the incorrect dose date.

Report Output

Each of the fields displayed in the output of the report are described below. See Understanding the Report Output Table for a definition of each column in the table.
