Reason for Termination or Transition Codes
Choose the reason that special education services were terminated for the student or the reason the student was transitioned from PK to school age special education services from the following options.

| 12 | Received Certificate |
| 13 | Reached Maximum Age of 22 |
| 14 | Deceased |
| 15 | Dropped |
| 16 | Moved, Known to be continuing in an Educational Program Only |
| 17 | Moved, but continuing Special Education services under IDEA in Indiana |
| 99 | Not eligible or no longer qualifies |

2 | Graduated with High School diploma |
3 | Received a Certificate |
4 | Reached Maximum Age of 22 |
5 | Deceased |
6 | Dropout |
7 | Moved, known to be continuing in an Education program ONLY (no longer receiving special education services) |
