Creating Incidents: Incident Descriptions

Use the steps below to populate the Incident Description portion of an incident. See the Appendix for code values.

1. Log into the school associated with the incident.
2. Navigate to Start Page > Special Functions > Incident Management Create New Incident.
3. Choose an incident type from the Incident Type pop-up menu.

The Incident Date and Time fields are populated based on when the incident occurred.

4. Choose the appropriate Time Frame (Before School Hours, During School Hours, After School Hours) from the pop-up menu.

The Time Frame Comment box appears.

5. If appropriate, enter a comment regarding the Time Frame (not required for state reporting).
6. Enter a description in the Title field, if applicable.
7. Choose the appropriate Location (District Conveyance, Off School Grounds, On School Property Grounds) from the pop-up menu.

Do one of the following:

      • If you choose District Conveyance, the Location subcode pop-up menu appears. Choose the appropriate value. The Location Comment box appears. If appropriate, enter a comment regarding the location (not required for state reporting).
      • If you choose Off School Grounds, the Location subcode pop-up menu appears. Choose the appropriate value. The Location Comment box appears. If appropriate, enter a comment regarding the location (not required for state reporting).
      • If you choose On School Property Grounds, the Location subcode pop-up menu appears. Choose the appropriate value. The Location Comment box appears. If appropriate, enter a comment regarding the location (not required for state reporting).
