Introduction

This file format pertains to the data submitted to ISBE health for collection of immunization/health examination data.

Selection Criteria

Student Selection

The report selects the student records to use based on the following criteria:

Students with an exit date before an entry date will not be included in the report.
This report only includes students who are included in State Reporting.
This report only includes students whose enrollments are from schools which are included in State Reporting.

Field	Description
Schools to Include	This report is run at the school level Current School Only – Includes all student records associated with the current school that meet selection criteria. To change the current school, click the School link at the top of the page and choose the appropriate school from the list.
Students to Include	Indicate which students to include in the report by selecting one of the following options: The selected [number] students only – Run the report for students in the current selection. All students – Run the report for all students in the current school, selected schools or district that meet the selection criteria. Note: If running the report for a single student, or group of students, select the students prior to running report.
Snapshot Date	Active Students with valid Records that are updated on or before this date.
Total Number of Students excluded from school (grade)	Enter the grade wise student to be excluded from the school. This will be printed in "Total number of students who were excluded from school for not being in compliance" section of the report. Numeric field positive integer

Report Output

Each of the fields displayed in the output of the report are described below. See Understanding the Report Output Table for a definition of each column in the table.

School Name	Name of school for which the report is run
School Year	The school year for which the report is run.
RCDTS	The RCDTS in this report is the District Number as defined in the prefs table for a district
Snapshot Date	The Snapshot Date in the report parameters

Polio (IPV/OPV)	IPV	Number of students in the grade for a school who are Protected and in Compliance as on Snapshot date and in same school year as that of snapshot date. A valid Exemption as on Snapshot date is also considered as Protected and in Compliance	Number of students in the grade for a school who are Not Protected and in Compliance for the reason Religious Objection, as on Snapshot date and in same school year as that of snapshot date. Actual unduplicated count of students who are unprotected and in noncompliance with immunization requirements. Count students only once, even if they are noncompliant in more than one disease category. They may also need a physical examination.
Grades	Pre-K to 12
Count of Students	Count students only once, even if they are noncompliant in more than one vaccine category. Students are counted based on Snapshot date and in same school year as that of snapshot date.
Total number of students who are in noncompliance with the physical exam requirement ONLY. Do NOT include any students counted in the actual unduplicated count of students in the row above
Grades	Pre-K to 12
Count of Students	If the Student who is entering a grade is ‘Out of Compliance’ for Grade Level Entry Certification but in compliance with all the Vaccines then the student will be counted as 1 Do NOT include any students counted in the actual unduplicated count of students in the row above. Students are counted based on Snapshot date and in same school year as that of snapshot date. If a student s in grade K, they need a grade level certification for grade K entered. If a student is in grade 1-5, they need a grade level certification for grade 1 entered. If a student is in grade 6-8, they need a grade level certification for grade 6 entered. If a student is in grade 9-12, they need a grade level certification for grade 9 entered.

Total number of students who are without physical examination only but compliant due to religious objection or Homeless Education Assistance/McKinney Vento Act only. Do NOT include any student counted in the actual unduplicated count of students in the row above.
Grades	Pre-K to 12
Count of Students	If for a Student entering a grade, the Student has ‘Compliance due to Religious Objection’ OR ‘Compliance under the McKinney-Vento Act’ for Grade Level Entry Certification and in compliance with all the Vaccines then the student will be counted as 1 in the report. Do NOT include any student counted in the actual unduplicated count of students in the row above. Students are counted based on Snapshot date and in same school year as that of snapshot date. If a student s in grade K, they need a grade level certification for grade K entered. If a student is in grade 1-5, they need a grade level certification for grade 1 entered. If a student is in grade 6-8, they need a grade level certification for grade 6 entered. If a student is in grade 9-12, they need a grade level certification for grade 9 entered.
Total number of students who were excluded from school on October 15, or an earlier established exclusion date, for not being in compliance. Report students excluded for one or more days.
