IL Eye Worksheet Report

Introduction

This file format pertains to the data submitted to ISBE health for the collection of Eye examination data.

Selection Criteria

Student Selection

The report selects the student records to use based on the following criteria:

  • Students with an exit date before an entry date will not be included in the report.

  • This report only includes students who are included in State Reporting.

  • This report only includes students whose enrollments are from schools which are included in State Reporting.

Description

Schools to Include

This report is run at the school level

  • Current School Only – Includes all student records associated with the current school that meet selection criteria. To change the current school, click the School link at the top of the page and choose the appropriate school from the list.

Students to Include

Indicate which students to include in the report by selecting one of the following options:

  • The selected [number] students only – Run the report for students in the current selection.

  • All students – Run the report for all students in the current school, selected schools or district that meet the selection criteria.

Note: If running the report for a single student, or group of students, select the students prior to running report.

Lunch Status, Low Income Indicator

Select this option to indicate low income and lunch status of the student.

Snapshot Date

Active Students with valid Records that are updated on or before this date. Default value 30th June <current year>

Report Output

Each of the fields displayed in the output of the report are described below. See Understanding the Report Output Table for a definition of each column in the table.

The report output will be in a PDF file with the name Eye_Worksheet_YYYYMMDD_XXXX , where date is the date of execution, and the last 4 digits are the incremental sequence number

Data Element

Description

School Name

Name of school for which the report is run 

School Year

The school year for which the report is run.

RCDTS

The RCDTS in this report is the District Number as defined in the prefs table for a district

Snapshot Date

The Snapshot Date in the report parameters

Data in the report will be displayed grade wise from Kindergarten to 12th grade. 

The data is extracted as on snapshot date provided by the user at report run time

The Vision and coloring Screening Date and Waiver Date is on or before the snapshot date and is within the 12 months as that of snapshot date.

Total number of Children

Total students enrolled in the grade as on snapshot date in total.

No of Children required to have an eye examination

Grade wise update of No of children entering the Kindergarten and No. of Children entering grades 1-12 in the Illinois School for the first time

No of Children completed eye examination

No of children grade-wise having Vision and Coloring Screening record and requiring eye examination

No. of children for whom a waiver is submitted because the child is enrolled in the free and reduced price lunch program and is ineligible for public insurance (Medicaid/All Kids)

Grade wise no. of Children whose Student demo tab → Free or Reduced Meal Benefits (Low Income Family) Indicator checked and requiring eye examination

No. of children for whom a waiver is submitted because the child is enrolled in Medicaid/All Kids, but is unable to locate a medical doctor who performs eye examinations or an optometrist in the community able to examine the child and who will accept Medicaid/All Kids

Grade wise no. of Children whose Student demo tab → Free or Reduced Meal Benefits (Low Income Family) Indicator checked and requiring eye examination

No. of children for whom a waiver is submitted because the child does not have any type of medical or vision/eye care insurance coverage and there are no low-cost eye/vision clinics in the community that will examine the child.

Grade wise no. of Children with Vision Waiver > Waiver Reason = ‘Undue Burden’ AND Student Health > Other > Has Health Insurance is unchecked and requiring eye examination

No. of children receiving an exemption based on religious objection.

Grade wise no. of Children with Vision Waiver > Waiver Reason = ‘Religious Objection’ and requiring eye examination.

No. of children submitting proof of an eye examination appointment scheduled to occur within 60 days after the October 15 deadline

Grade wise no. of Children with Vision Waiver > Waiver Reason = ‘Vision appointment Proof provided and requiring eye examination.

No. of children with low income based on their lunch status (if selected by user)

Students with low income based on their lunch status (if selected by user).

No. of children not completing eye examinations (no waiver, no religious exemption, and no approved eye examination appointment scheduled to occur within 60 days after October 15).

Students with no Vision and Coloring Screening and Waiver data and the as per snapshot date and the Free or Reduced Meal Benefits (Low Income Family) Indicator not checked but requiring eye examination
