IL Dental Worksheet Report

Introduction
This file format pertains to the data submitted to ISBE health for the collection of Dental examination data.
Selection Criteria
Student Selection
The report selects the student records to use based on the following criteria:
- Students with an exit date before an entry date will not be included in the report.
- This report only includes students who are included in State Reporting.
- This report only includes students whose enrollments are from schools which are included in State Reporting.

Students to Include | Indicate which students to include in the report by selecting one of the following options:
Note: If running the report for a single student, or group of students, select the students prior to running report. |
Snapshot Date | Active Students with valid Records that are updated on or before this date. |
Report Output
Each of the fields displayed in the output of the report are described below. See Understanding the Report Output Table for a definition of each column in the table.

| School Name | Name of school for which the report is run. |
| School Year | The school year for which the report is run. |
| RCDTS | The RCDTS in this report is the District Number as defined in the prefs table for a district. |
| Snapshot Date | the Snapshot Date in the report parameters |
Data in the report will be displayed as per the student's race/ethnicity for grades - Kindergarten, Second, Sixth and Ninth. As required in the report template
Data will be dynamically generated for the student race/ethnicity as given in the table below.
Ensure the Races defined in the Dental Worksheet report matches the District Race code Description.
