Introduction

A 504 Plan identifies a temporary disability that may inhibit learning. Districts are required to report 504 Plans by April 16 each year, continuing to keep the 504 Plan up to date throughout the student’s
enrollments.

Field	Description
Schools to Include	Choose one of the following: Current School Only – Includes all student records associated with the current school that meet selection criteria. To change the current school, click the School link at the top of the page and choose the appropriate school from the list. All Schools – Includes all student records on the PowerSchool server that meet selection criteria. Note: The All Schools option is only available when running the report from the District Office.
Current Selection Students	Indicate which students to include in the report by selecting one of the following options: The Selected [Number] Students Only – Run the report for students in the current selection. All students – Run the report for all students in the current school or district that meet the selection criteria. Note: If running the report for a single student, or group of students, select the students prior to runningreport.
RCDTS Number (Override)	Enter the 15-digit RCDTS (Region, Country, District, Type, and School) number. The value entered in this field will be used in the file name and the header in the report results.
Snapshot Date	Enter the snapshot date for the report.

Each of the fields displayed in the output of the report is described below. See Understanding the Report Output Table for a definition of each column in the table.

This field should be blank for all students upon first submission of the student. 6	RCDTS Home School	The RCDTS number that identifies the student’s home school. Extracted from the Demo tab of the Illinois State Information page, Start Page > Student Selection > State/Province - IL > ISBE SIS Student Demographic/Enrollment Information. 7	RCDTS Serving School	The RCDTS number that identifies the school that provides service to the student. Extracted from the Demo tab of the Illinois State Information page, Start Page > Student Selection > Illinois State Information >ISBE SIS Student Demographic/Enrollment Information. If the RCDTS Serving School is blank for a student, the RCDTS Home School entered on the Edit School page is used in the report. If the RCDTS Serving school is the same as a Concurrent Enrollment School, a record is created for the RCDTS Serving School. 8	School Year	The school year reported. This field is derived from the selected Snapshot Date in the report parameters.	Derived from parameters	Date	4
9	504 Plan Start Date	The date the student's 504 plan started. Extracted from the 504 Plan tab of the Illinois State Information page, Start Page > Student Selection > State/Province - IL > 504 Plan. If no 504 Plan Start Date is set, the report extracts the student's enrollment entry date. If the RCDTS Serving School is set to a Concurrent Enrollment School, the report extracts the plan start date from the concurrent enrollment. 10	504 Plan End Date	The date the student's 504 plan ended. Extracted from the 504 Plan tab of the Illinois State Information page, Start Page > Student Selection > State/Province - IL > 504 Plan. If populated, the 504 Plan End Date overrides the student's enrollment Exit Date. If no 504 Plan End Date is set and the enrollment Exit Date does not fall before the snapshot date, the report extracts the date based on the following: If the 504 Plan Start Date falls on or prior to the report snapshot date, the report extracts the last in-session day prior to the student's enrollment Exit Date. If the 504 Plan Start Date falls after the snapshot date, the 504 Plan End Date displays as blank. If the RCDTS Serving School is set to a Concurrent Enrollment School, the report extracts the plan end date from the concurrent enrollment.
