Washington School Immunizations Report
Introduction
The School Immunizations report is used to gather student immunization information. The report uses the State Reporting Platform (SRP), which optimizes the performance of the report.
The report determines the students' status according to the Washington State Department of Health vaccine requirements. This report can be used to identify the number of exempt students by category, and by vaccination, the number of students complete or immune, the number out of compliance, and the number of student exempt, conditional or out of compliance by vaccine. This report may be run at the school or district level and may take several hours to run depending on the size of your school or district.
Running the School Immunizations report will generate the output files, which are in a zip format: WA_School_Immunizations_report.zip. When unzipped, there are two files:
- Immun_Audit. This is the CSV file listing of each student included in the report and their associated information.
- Immun_Summary. This is the PDF file displaying the immunizations totals by school.
Selection Criteria
Selection criteria determine which database records are used in the report. Refer to selection criteria for analysis of report results when the report does not return the correct records.
Student Selection
The report selects student records from the [Students] and [ReEnrollments] table based on the following criteria:
- The school must not be excluded from state reporting.
- The student must not be excluded from state reporting.
The student must be enrolled in a school included in the report.
- The student's school Entry Date must occur on or before the report date.
- The student's school Exit Date must occur on or after the report start date.
- The student must not be a no-show; i.e., their school Exit Date must be after their Entry Date.
