Incident Management Validation Extract File Report
Introduction
This report is a validation report based on events entered via the Incident Management module. It lists incomplete incident records. An incomplete record has specific attributes that can be checked by the validation report. The report will generate a list of incidents that have one or more of the following errors:
- The incident has an offender with no Behavior incident element attached.
- The incident has an offender with no Action incident element attached.
- The incident does not have an Incident Date.
Selection Criteria
Selection criteria determine which database records are used in the report. Refer to selection criteria for analysis of report results when the report does not return the correct records.
Student Selection
The report selects records from the Incident Management module tables based on the following criteria:
- The incident occurred during the school year set at the top of the page.
School Selection
The report selects records from the [Schools] table that are not excluded from state reporting.

Select Schools | If run at the district level, choose one of the following:
If run at the school level, the report includes all records associated with the current school that meet the selection criteria. To change the current school, click School at the top of the page and choose the appropriate school from the list. |
Current Selection Students | Indicate which students to include in the report by selecting one of the following options:
Note: If running the report for a single student, or group of students, select the students prior to running report. |
School Year | The choice presented is set by the term at the top of the page. |
Report Output
Each of the fields displayed in the output of the report are described below. See Understanding the Report Output Table for a definition of each column in the table.
Note: If there are no validation errors the message "No Records Found" will output.

1 | Incident ID | The validation error message associated with the student. The OSPI validation message wording is simplified to better relate to PowerSchool terminology. For example, the report returns 'NO DOB' instead of 'Required Field: DistrictStudent.BirthDate (element B09) must be a date in the format MM/DD/YYYY.' |
2 | Incident Title | [Schools]Name. Name of school where the student is enrolled. |
3 | Incident Date | [Schools]Abbreviation. School abbreviation. |
4 | Offenders Count | Total number of offenders associated with the incident. |
5 | Error-NoBehavior-ForOffender | List of offenders that have no behavior element attached. When the incident has multiple offenders with this error the offenders are separated by ";". The format is student number–student full name. |
6 | Error-NoAction-ForOffender | List of offenders that have no action element attached. When the incident has multiple offenders with this error the offenders are separated by ";". The format is student number–student full name. |
7 | Error-NoIncidentDate | "Y" for incidents that have no date associated with the incident. |
