Immunizations Report

Introduction

This report extracts immunization and compliance information for students in a specific grade level, in a PDF file. The immunization and immunization compliance data that is extracted is based on the user defined snapshot date

Selection criteria determine which database records are used in the report. Refer to selection criteria for analysis when the report does not return the correct records.

Selection Criteria

Student must be active on the user defined snapshot date.
Student must be in the grade level that is selected at report runtime.

Description

Snapshot Date

Enter the snapshot date for the report.

Select Grades

Select the grade level that you wish to run the report for. The report can only be run for one grade level at a time.

Note: The grade levels that are displayed are based on the grade levels that the currently selected building serves.

Each of the fields displayed in the output of the report are described below. See Understanding the Report Output Table for a definition of each column in the table.
