Health Service Tab UI Update for SHARRS

Following updates are made to the Health service tab to accommodate the changes for the SHARRS report:

1. The label, "Office Visit due to Acute/Chronic Illness?" is changed to "Office Visit due to Chronic Illness?"

2. The Select Chronic Illness: field is moved after "Office Visit due to Chronic Illness?"

3. A new field is displayed with the label “Office Visit due to acute illness?” with yes/no options. The field is listed under "Select Chronic Illness:"
