The following Section 504 disability codes must be assigned at the student level.

13 | Chronic Fatigue Syndrome |
14 | Migraine Headaches |
15 | Broken (expected 6+ months duration) or Missing Body Part |
16 | Eye Abnormality or Vision Impairment |
