Creating Incidents: Incident Builder: Incident Elements

Introduction

Use the steps below to add behaviors, actions and attributes to the incident. See the Appendix for code values.

Add a BEHAVIOR – The type of incident

  1. Click the Plus (+) button next to Incident Elements.
  2. Choose Add Behavior from the pop-up menu.
  3. Choose the appropriate behavior (Misconduct Against a Person^, Misconduct-Against-Society^, etc.) from the pop-up menu. A behavior subcode pop-up menu appears.
  4. Choose the appropriate subcode from the pop-up menu.
    Note: The Primary Behavior checkbox is not required for PA State Reporting purposes.
  5. Click Add Behavior. The behavior appears under the Incident Elements heading.
  6. Click and drag the behavior to the appropriate offender. The behavior appears under the name of the offender and also remains listed under the Incident Elements heading.

Important: The Behavior Element may be attached to multiples offenders. If a Behavior Element is not attached to at least one offender it will not be reported.

Add an OBJECT – If the offender was in the possession of a weapon

  1. Click the Plus (+) button next to Incident Elements.
  2. Choose Add Object from the pop-up menu.
  3. From the pop-up menu next to Object Code choose a code (i.e ILLEGAL POSSESSION OF A WEAPON^).
  4. Choose an applicable subcode from the pop-up menu below.
  5. If the selected subcode has a comment section, complete the Object Code Comment.
  6. Enter the Quantity (number of weapons) Note: This is required for Pennsylvania State Reporting.
  7. Click and drag the object to the appropriate Offender. The Object appears under the name of the offender and also remains listed under the Incident Elements heading.

IMPORTANT- if a Object Element is not attached to an offender it will not be reported.

Add an ACTION – The Discipline Action taken against the Offender

  1. Click the Plus (+) button next to Incident Elements.
  2. Choose Add Action from the pop-up menu.
  3. Choose the appropriate Action Code (i.e. State Sanction Valid Values^ or State Remedial Program Values^) from the pop-up menu. An action subcode pop-up menu appears.
  4. Choose the appropriate subcode from the pop-up menu.
  5. In the Action Taken Detail field, enter any comments related to the Disciplinary Action taken.
  6. From the Duration Code pop-up menu, choose DAYS^ .
  7. Enter the Assigned Duration and Actual Duration. This is required for PA State Reporting.
  8. Complete all fields under ‘Action Attributes’ as required by PIMS-Discipline Domain.
  9. Click the Add Action button located at the bottom of the page.  The action appears under the Incident Elements heading.
  10. Click and drag the action to the offender, which should be listed under Participants. The action appears under the behavior and is no longer listed under the Incident Elements heading.

IMPORTANT: If an Action Element is not attached to an offender, it will not be reported.

Add an ATTRIBUTE

Note: Attributes are applied to the entire incident and reside under Header of Incident Elements. They cannot be moved to a participant.

  1. Click the Plus (+) button next to Incident Elements.
  2. Choose Add Attribute from the pop-up menu.
  3. Select the appropriate checkbox. For example, if the incident drug related, select the checkbox next to the Drug related indicator.
  4. Click the Add Attributes button at the bottom of the page
