California School Immunization Record

Introduction

The California School Immunization Record (Blue Card) report is used to track student immunization history.

Selection Criteria

Selection criteria determine which database records are used in the report.

Student Selection

The report selects records from the [Students] or [ReEnrollments] table based on the Report Input. The report returns students regardless of whether the student has an immunization record. Also, the Exclude from State Reporting flag is ignored.

If the report is run for all students in the school, only active students are returned. If the report is run for a current selection, then all students in the selection are returned, regardless of whether they are currently active at the school.

This report is run at the school level only.

Field

Description

Students to Include

Indicate which students to include in the report by selecting one of the following options:

  • The selected [number] students only – Run the report for all students (active and inactive) in the current selection who meet the selection criteria.

  • All students – Run the report for all active students in the current school who meet the selection criteria.

Notes:

  • If running the report for a single student, or group of students, select the students prior to running the report.

  • This report must be run for individual schools; the report cannot be run for the district.

Report Student Name as

Choose either Legal Name or Preferred Name.

Note:

The display of this parameter and its values are dependent on the configurations made to the California School Immunization Record on the Report Parameter Authorizations page (District ->System Administrator -> Report Parameter Authorizations)

Include Instructions page

Choose Yes or No to indicate whether the instructional page should be printed along with the immunization card.

Each of the fields displayed in the output of the report is described below. See Understanding the Report Output Table for a definition of each column in the table.

Note:  All students are evaluated and reported based on the requirements for their current grade level.

 For school year 23-24 or higher, grade level mapped to LTK will be displayed. 

This report is output as a PDF.

Vaccination Details

For some immunizations, the required number of doses varies depending on the student's age at the time that a particular vaccine was administered. For these vaccines, an age indicator field is displayed for the applicable dose(s). The value displayed is a calculation of the student's age on the date entered for the vaccine dose(s).

IPV / OPV

The date that each vaccine dose was administered, and an indicator of whether the student has a permanent medical exemption for this vaccine.

Age calculated based on the date immunization was administered.

Status requirements are grouped by grade levels according to California's immunization admission requirements.


Staff Initials

The initials of the staff member who reviewed the student's immunization record.

The date the follow-up is needed.

Other

Indicates that the student does not have all of the required immunizations, does not meet the criteria for conditional admission, and is:

  • accessing special education services required by the student's individualized education program (IEP), or

  • enrolled in an independent study program and does not receive classroom-based instruction (IND), or

  • enrolled in a home-based private school (Home), or

  • transferring from another school in California with a valid personal beliefs exemption filed before 2016 (PBE).
