Introduction

This template defines the primary details of a discipline incident. One record should be submitted per each reportable School Safety incident. Grain: One record per Submitting District Code / School Year Date / Incident ID

Selection Criteria

Selection criteria determines which database records are used in the report. For the Incident Template, students' discipline incident records are selected.

School Selection

The report selects records from the Schools table where at least one incident record occurred at the school during the reporting period.

Student Selection

  • The report selects the records from the Students table where student is not excluded from state reporting.

Discipline Incident Selection

  • The report selects the records from the Incident Management Tables where the Incident Type is State Reportable.

Description

Select Schools*

Indicate which schools to include in the report by selecting from the following options:

If sub-districts are not enabled:

  • Select Multiple Schools – Select the schools for which you want to run the report. Includes all student records associated with the selected schools that meet the selection criteria.
  • All Schools (default) – Run the report for all schools. Includes all student records on the PowerSchool server that meet the selection criteria

If sub-districts are enabled: 

  • Select Sub-district - Select the sub-district for which you want to run the report.
  • Select Multiple Schools – Select the schools for which you want to run the report. Includes all student records associated with the selected schools that meet the selection criteria.
  • All Schools in selected sub-district (default) – Run the report for all schools in the selected sub-district. Includes all student records on the PowerSchool server that meet the selection criteria.

If run at the school level, the report includes all records associated with the current school that meet the selection criteria. To change the current school, click School at the top of the page and choose the appropriate school from the list.

Note: For Current School Only - Only incidents that OCCURRED at the Current School will be included; thus, students from another school may be included. It will NOT include students from the Current School who were involved in an incident at another school. For example, if John Smith from East High School was involved in an incident at West High School, he will be reported when running the report for West High School, NOT East High School.

Students to Include

Indicate which students to include in the report by selecting one of the following options:

  • The selected [number] students only – Run the report for students in the current selection.
  • All students – Run the report for all students in the current school or district that meet the selection criteria.

Note: If All Students is selected, then incidents involving Staff or Others are included. If Selected Students is selected, then incidents involving Staff or Others are excluded.

Select School Year

Select the applicable school year for which the data is needed.

Enter the start date for the reporting period. Incidents must occur on or after this date)*

Each of the fields displayed in the output of the report is described below. See Understanding the Report Output Table for a definition of each column in the table.

Submitting AUN

The unique 9-digit Administrative Unit Number assigned by the PDE. This field is extracted from Start Page > District Setup > District Information.

School Year Date

The last day of the school year. This date is calculated based on the [Terms]LastDay field.

School Number of Incident

The PDE code that identifies the school where the incident took place. This value is extracted from Start Page > District Setup > Schools/School Info > Edit School.

If the PIMS Location Code is blank on that page, then the School Number associated with the Incident is reported.

Note: If Location Code is other than 1, the Incident Time Frame is reported as blank.

Local Incident ID

The database ID automatically assigned to the incident by PowerSchool.
