Incident Offender Template

Introduction

This template connects one or multiple offenders to each specific incident. If there are multiple offenders, include multiple records within this template with the same Incident ID. The template generates records of 15 fields, for each Submitting District Code per Incident ID per Offender ID per School Year Date. Incident Template and Person Template must be submitted prior to this report. Offender Id on this report matches Person Id from the Person Template.

This report pulls incidents with Behavior codes 'A - Academic Code of Conduct' or 'C - Student Code of Conduct'.

Selection Criteria

Selection criteria determines which database records are used in the report.

School Selection

  • At least one incident record must have occurred at the school during the reporting period.

Student Selection

  • Student must not be excluded from state reporting.

Discipline Incident Selection

  • The Incident Type must be State Reportable.

Description

Select Schools

Indicate which schools to include in the report by selecting from the following options:

If sub-districts are not enabled:

  • Select Multiple Schools – Select the schools for which you want to run the report. Includes all student records associated with the selected schools that meet the selection criteria.

  • All Schools (default) – Run the report for all schools. Includes all student records on the PowerSchool server that meet the selection criteria

If sub-districts are enabled: 

  • Select Sub-district - Select the sub-district for which you want to run the report.

  • Select Multiple Schools – Select the schools for which you want to run the report. Includes all student records associated with the selected schools that meet the selection criteria.

  • All Schools in selected sub-district (default) – Run the report for all schools in the selected sub-district. Includes all student records on the PowerSchool server that meet the selection criteria.

If run at the school level, the report includes all records associated with the current school that meet the selection criteria. To change the current school, click School at the top of the page and choose the appropriate school from the list.

Note: If Current School Only is select, only incidents that occurred at the current school will be included; therefore, students from another school may be included. Students from the current school who were involved in an incident at another school will not be included. For example, if John Smith from East High School was involved in an incident at West High School, he will be reported when running the report for West High School, but not East High School.

Students to Include

Indicate which students to include in the report by selecting one of the following options:

  • The selected [number] students only – Run the report for students in the current selection.

  • All students – Run the report for all students in the current school or district that meet the selection criteria.

Note: If All Students is selected, then incidents involving Staff or Others will be included. If Selected Students is selected, then Incidents involving Staff or Others will be excluded.

Select School Year

Select the applicable school year for which the data is needed.

Report Start Date

Enter the start date for the reporting period – (Incidents must occur on or after this date)

Report End Date

Enter the end date for the reporting period – (Incidents must occur on or before this date)

The age of the student at the time the incident occurred. Calculated based on the student’s date of birth and the incident date.

Grade Level Code at Time of Incident

The grade level of the student at the time the incident occurred. Calculated based on the incident date and the student’s school enrollment during the reporting period (current or previous enrollment, whichever was active during the reporting period and on the incident date).

Both the grade level and grade code fields are extracted from Start Page > Student > Transfer Info > Edit Enrollment.

Indicates whether local law enforcement was contacted regarding this incident.

Adjudication Code

The type of adjudication of the offender for the incident. Includes the Adjudication Comment if one has been stored.

Extracted from Start Page > Student > Incidents.

  • 2-Convicted as Adult

  • 7-Withdrawal/Dismissal of Charges

  • 8-Found Not Guilty

Weapon Detected Method Code

A description of how the weapons involved in the incident for the offender were detected. Required if Field 10 - Arrested Code is populated with YW.

  • 4 – Detected by other adult visitor

Offender District Of Residence

The district Administrative Unit Number (AUN) of the Charter school or District of Residence that sent the student to the current LEA. 

This value is extracted from the student's District of Residence field. If there is no value available in the District of Residence field or if the Reporting District Type is CD, the District AUN is extracted.

Note: Required only if Field 5 - Offender Type is 1, 2, or 5; otherwise blank.
