Incident Offender Disciplinary Action Template

Introduction

This template is used to track disciplinary sanctions, as well as referral to or participation in remedial programs. Each known offender should have only one sanction record in this template identifying the "highest" or most severe sanction (Disciplinary Action Code between S1 and S10). If referred to or participating in remedial programs, the offender will also have one or multiple additional records in this template with the Disciplinary Action Code between R1 and R9.

This report pulls incidents with Behavior codes 'A - Academic Code of Conduct' or 'C - Student Code of Conduct.

Note: If the offender is unknown for an incident, do not submit a record in this template.

Grain: One record per Submitting District Code / Incident ID* / Offender ID / Disciplinary Action Code / School Year Date

*Denotes per incident id which means that an offender is allowed to have more than one qualifying record in the template.

Selection Criteria

Selection criteria determines which database records are used in the report.

School Selection

  • The report selects records from the Schools table where at least one incident record occurred at the school during the reporting period.

Student Selection

  • The report selects the records from the Students table where student is not excluded from state reporting.

Discipline Incident Selection

  • The report selects the records from the Incident Management Tables where the Incident Type is State Reportable.

Description

Select Schools*

Indicate which schools to include in the report by selecting from the following options:

If sub-districts are not enabled:

  • Select Multiple Schools – Select the schools for which you want to run the report. Includes all student records associated with the selected schools that meet the selection criteria.
  • All Schools (default) – Run the report for all schools. Includes all student records on the PowerSchool server that meet the selection criteria

If sub-districts are enabled: 

  • Select Sub-district - Select the sub-district for which you want to run the report.
  • Select Multiple Schools – Select the schools for which you want to run the report. Includes all student records associated with the selected schools that meet the selection criteria.
  • All Schools in selected sub-district (default) – Run the report for all schools in the selected sub-district. Includes all student records on the PowerSchool server that meet the selection criteria.

If run at the school level, the report includes all records associated with the current school that meet the selection criteria. To change the current school, click School at the top of the page and choose the appropriate school from the list.

Note: If selecting Current School Only, only incidents that OCCURRED at the Current School will be included; thus, students from another school may be included. It will not include students from the Current School who were involved in an incident at another school. For example, if John Smith from East High School was involved in an incident at West High School, he will be reported when running the report for West High School, not East High School.

Students to Include*

Indicate which students to include in the report by selecting one of the following options:

  • The selected [number] students only – Run the report for students in the current selection.
  • All students – Run the report for all students in the current school or district that meet the selection criteria.

Note: If All Students is selected, then incidents involving Staff or Others will be included. If Selected Students is selected, then Incidents involving Staff or Others will be EXCLUDED

Select School Year*

Select the applicable school year for which the data is needed.

Report Start Date

Enter the start date for the reporting period – (Will report offenders on incidents on or after this date)*

Report End Date

Enter the end date for the reporting period – (Will report offenders on incidents on or before this date)*

The number that identifies this discipline incident.

Disciplinary Action Code

The code of the sanction taken against the offender for the incident or the remedial program the offender has been referred to or is participating in.

Sanction Valid Values:

• S1 – none
• S2 – detention
• S3 – in school suspension
• S4 – out of school suspension
• S5 – expulsion, less than one calendar year
• S6 – expulsion, one calendar year
• S7 – expulsion, more than one calendar year
• S8 – special education student removed to an interim alternative educational setting by school personnel
• S9 – special education student removed to an interim alternative educational setting by due process hearing officer
• S10 – other

Remedial Program Valid Values:

• R1 – alternative education
• R2 – home study instruction
• R3 – student assistance referral
• R4 – drug/alcohol counseling
• R5 – guidance counseling
• R6 – psychological evaluation
• R7 – peer mediation / conflict resolution
• R8 – anger management
• R9 – other

A single date indicating the whole school year; June 30 at the end of the standard school year is used to designate a school year. Thus, the 2016-2017 school year would be indicated by 2017-06-30
