Oral Health Assessments/Waivers

Introduction

The Oral Health Assessments/Waivers report includes oral health record results for kindergarten and new first grade students.

Selection Criteria

Selection criteria determine which database records are used in the report.

Student Selection

The report selects which student records from the [Students] or [ReEnrollments] table based on the following criteria:

  • The student must not be excluded from state reporting.

  • The student must be enrolled in a school included in the report. The school must not be excluded from state reporting and must not be designated as a summer school.

  • The student must have an Oral Health Assessment or Waiver record.

Notes:

    • If a student has multiple Oral Health records, the report refers to the most recent record. Assessment records are given first priority, Waiver records second priority, No Form Returned records last priority.

    • Oral Health Assessment records must occur within one year prior to the start date of the current school year through the current school year to be included in the report.

    • Oral Health Waiver records must occur within the school year chosen from the pop-up menu at report runtime.

    • This parameter is ignored for the total kindergarten student count.

  • The student must be enrolled in grade level 0 (kindergarten) or 1 (first).

  • The student must be active in the school; i.e. the value for the Enroll_Status field in the [Students] table should be 0.

Note: If Yes is selected for include inactive students at report runtime, the Enroll_Status field must equal 2 (transferred out). Only inactive students that were active at some point during the current school year are included in the report.

This report is run at the District level.

Data to be Filled

Select the checkbox next to the filter fields to save the settings as defaults. To select all checkboxes, choose Set All from the pop-up menu. To clear all checkboxes, choose Reset All.

Include Inactive Students

Choose Yes or No from the pop-up menu. Choosing Yes includes students who are currently enrolled and students who are transferred out ([Students]Enroll_Status = 0 or 2).

Each of the fields displayed in the output of the report are described below. See Understanding the Report Output Table for a definition of each column in the table.

Kindergarten Students

The number of active kindergarten students.

This number may include inactive students, if selected at report runtime. The report only includes inactive students who were active at some point during the school year.

Note: This total includes students with and without an Oral Health Assessment or Waiver record.

New First Grade Students

The number of first grade students with an Oral Health Assessment or Waiver record who did not attend kindergarten at a public school. This number may include inactive students, if selected at report runtime.

Note: The report validates that the student did not attend kindergarten (grade level 0) within the district for any prior school year. However, students are considered New First Grade Students if they are promoted from kindergarten to first grade during the school year.

The report adds the totals for kindergarten and new first grade students.

The number of Oral Health Assessment records for the selected students.

Oral Health Assessment records must have a date that occurs within a year prior to the school start date to the report end date.
