Introduction
The Immunization Rate Percent report shows the percentage of students who have all the vaccines required for their grade level as of the report date and the percentage of students with non-medical exemptions. This report provides a different view of the same data that is in the Immunization Primary Review Summary report; however, the values in this report are displayed in a bar graph for each vaccine, with the percentage values below the graph. The report can be run for one or multiple schools. When run for multiple schools, the report returns one page per school. Schools with no results are not included in the output.
The report has two output types available: K-12 and Pre-school. The K-12 output displays a graph with the percentage of students in grades K-12 who have complete or up-to-date vaccines for DTaP/Tdap, Polio, Varicella, Measles, Mumps/Rubella, HepB, and HepA. The Pre-school output includes the percentage for students in grades below Kindergarten who have complete or up-to-date vaccines for DTaP, Polio, Varicella, MMR, HepB, HepA, and HIB.
Depending on the date the report is run, the percentages in the report output might not total 100% because some students could have medical exemptions, or vaccinations that are incomplete or in progress but still on schedule.
Not all vaccinations are required for all grades at a school; for example in an elementary school with preschool students, the K-12 students do not require the HIB vaccine. When a vaccine is not required or is required for fewer than 10 students, the bar graph is blank for that vaccine.
IMPORTANT: To ensure accurate results, run the Immunization Audit Report before running this report.
Selection Criteria
The report selects records from the [Students] and [Reenrollments] tables based on the following criteria:
- The student must have a current or previous school enrollment at the appropriate school on the student inclusion date.
- The student must not be excluded from state reporting.
- The student's school must not be excluded from state reporting
