S - Students Restrained or Isolated File 2023-2024


This file contains information about students who are restrained or isolated and any staff injured during those incidents of students restrained or isolated. Restraint is defined as physical intervention or force used to control a student, including the use of a restraint device to restrict a student’s freedom of movement. It does not include the appropriate use of a prescribed medical, orthopedic, or therapeutic device when used as intended, such as to achieve proper body position, balance, or alignment, or to permit a student to safely participate in activities. Physical restraint does not include the touching or holding of a student without the use of force for the purpose of directing or assisting the student in an activity. The term physical restraint does not include a physical escort which is temporary touching or holding of the student's hand, wrist, arm, shoulder, or back for the purpose of inducing a student to walk to a safe location.

Selection Criteria

Selection criteria determine which database records are used in the report. Refer to selection criteria for analysis of report results when the report does not return the correct records.

Student Selection

The report selects records from the [Students] table based on the following criteria:

  • The school must be included in the report.
  • The school must not be excluded from state reporting.
  • The student must not be excluded from state reporting or from CEDARS reporting.
  • The student must be enrolled in a school included in the report.
  • The student's Incident Date must have occurred after or on the Report Start date and before or on the Report End Date.
  • The student's incident must have an incident Action Code of RES (Restraint) or ISO (Isolation)

Select Schools

Choose one of the following:

  • Current School Only – Includes all student records associated with the current school that meet the selection criteria. To change the current school, click the School link at the top of the page and choose the appropriate school from the list.
  • All Schools – Includes all student records on the PowerSchool server that meet selection criteria.

The All Schools option is only available when running the report from the District Office.

Current Selection Students

Indicate which students to include in the report by selecting one of the following options:

  • The Selected [Number] Students Only – Run the report for students in the current selection.
  • All Students – Run the report for all students in the current school or district that meet the selection criteria.
If you are running the report for a single student, or a group of students, select the students before running the report.
School YearChoose the year for which to run the report.


Select Schedule to run the report on a specific date and time. The default setting is 'Run Now.'

Report Output

Each of the fields displayed in the output of the report is described below. See Understanding the Report Output Table for a definition of each column in the table.

S01School Year

The four-digit year in which the school year ends.





S02County District Code

The County District code for the district submitting the data.






S03District Student ID

The locally assigned student identifier.





S04State Student ID (SSID)

The identifier assigned to each WA public school student by OSPI.

Rule: This field is not required if the student has not yet been assigned an SSID.





S05Location ID

The Location ID for the school. The school number is reported.





S06Action IDThe unique ID that identifies the action.Incident_Action.Incident_Action_IDYesVarChar20
S07Action Date

The date the action occurred.

MM/DD/YYYY format.

S08Action Duration

The approximate length of time in minutes that the student was restrained or isolated.

Valid values: 0.5to 999.5

S09Type of Restraint or IsolationThe type of restraint or isolation used during this action.

Valid Values:

  • 1 – 1 person restraint (basket, etc.)
  • 2 – 2+ person floor restraint
  • 3 – 2+ person seated restraint
  • 4 – 2+ person standing restraint
  • 5 – 2+ person wall restraint
  • 6 – Handcuffs (performed by law enforcement, no arrest made)
  • 7 – Handcuffs (performed by non-law enforcement)
  • 8 – Other Restraint - Physical
  • 9 – Weighted Blankets
  • 16 – Walking restraint/escort/transport
  • 17 – Handcuffs (performed by law enforcement, arrest made)

  • 18 – Other Restraint - Mechanical

  • 10 – Bus/Car/Other Vehicle
  • 11 – Classroom
  • 12 – Closet, locker room or other small space
  • 13 – Designated Isolation room
  • 14 – Office including nurse, counselor, or other
  • 15 – Other Isolation

S10Was the student injured during the action?

An indicator used to identify if the student was injured during the action in
which they were restrained or isolated.

Valid Values:

  • N – No
  • Y – Yes

S11How many staff were injured during the action?

The number of staff injured during the reported action of restraint or isolation.

Rule: If no staff is injured, the report extracts zero.

S12Safety and Security Staff Involvement Status

This element indicates the involvement of the safety or security staff.

All instances of school safety or security staff member involvement are reported, regardless of the action reported in Element S09 – Type of Restraint or Isolation.

The corresponding action attribute while defining the incident is - Safety or security staff were involved

Valid values:

  • 0 – Safety or security staff were not involved (attribute not selected)

  • 1 – Safety or security staff were involved (attribute is selected)

This field will report a blank value.



S14Safety and Security Staff Involvement Outcome for Student

This field will report a blank value.
