Introduction

This report is used to maintain State Student Identifiers (SSID) and basic student demographic information in the USOE database. This extract is created for active students based on student selection, and can be created at the district level or for a single school, and for all students or a student selection. The extract is a comma delimited text file.

The UT Unique ID Request File Report is uploaded to the USOE.

New IDs – A student enrolling for the first time in the state of Utah, including kindergarten students who do not have an SSID in the State Student Number field
Retrieve – A student or a selection of students who have transferred from another school in a Utah district
Update – A student whose attributes need to be updated based on a student SSID, LEA number and student number
Verify – A student or selection of students whose SSIDs need to be verified

Note: The report output is restricted to students enrolled in the current year.

Field	Description
Select Schools	Choose a sub-district from the Select Sub-district pop-up menu. Then select one of the following: Select Multiple Schools – Choose specific schools in the sub-district for which to run the report. All Schools in selected sub-district – Includes all student records in the sub-district that meet the selection criteria. Note: The All Schools option is only available when running the report from the District Office.
Select Students	Indicate which students to include in the report by selecting one of the following options: The Selected [number] Students Only – Run the report for students in the current selection enrolled in the specified date range. All students – Run the report for all students in the current school enrolled in the specified date range. Note: If running the report for a single student, or group of students, select the students prior to running report.
Type of report	Choose one of the following required options from the pop-up menu: New IDs – To obtain a new SSID for any student in PowerSchool who does not have an SSID in the State Student Number field. Retrieve – To obtain the SSID from the state system of a student or a selection of students who have transferred from another school in a Utah district. Update – To update a student’s attributes based on a student SSID, LEA number and student number. Verify – To confirm a student or selection of students SSIDs.
Include Inactive and Transferred Out Students	Select the checkbox to include inactive and transferred-out students in the report.
