Incident Management Verification Report

The Incident Management Verification Report extracts all Discipline records entered in Incident Management that are, or may be, state-reportable. This report can be used as verification for the PEIMS TX Interchange Student Discipline Extension, and will allow districts to identify records with missing or incorrect data entry that may cause the records to be omitted from the Discipline Interchange file. 

All key incident management fields, including the Texas-specific discipline fields, are extracted in .CSV format for the date range specified at runtime. In addition, a series of test checks are applied against specific data elements; these test checks appear in the final columns of the report. Any data element test column displaying a value of “missing” or "missing/invalid" indicates that the record does not meet the minimum data entry requirements for submission to TEA in the Student Discipline Interchange. Users should verify each item flagged as missing the test check, and then update or correct the data entry as needed in all Incident Management records that are reportable to the state.

Selection Criteria

Selection criteria determine which database records are used in the report.

The report selects Incident Management records based on the following criteria:

  • The incident must must be associated with a school that is included in the report.
  • The incident must must be associated with a student that is included in the report.
  • The incident date must fall within the Start Date and End Date range specified by the user at runtime.

Report Input

Follow the list of steps to locate and generate the report.

  1. On the Start Page, click State Reports. The Reports page appears.
  2. Click Incident Management Verification Report. The Incident Management Verification report page appears.
  3. Enter information in all of the required fields.
  4. Click Submit to generate the report.

Field

Description

Select Schools

If run at the district level, choose one of the following:

  • Select Multiple Schools – Select schools to include in the report. To select a single school, click that school. To select multiple schools, use Ctrl+Click and select each school to be included.
  • All Schools – Run the report for the entire district.

If run at the school level, the report includes all records associated with the current school that meet the selection criteria. To change the current school, click School at the top of the page and choose the appropriate school from the list.

Select Start and End Dates

Enter the date range to be used for the report

Incidents to Include

Select one option:

  • Include Only incidents with state behavior or action code
  • Include All Incidents
Include Comments

This option is now obsolete; comments are excluded by default.

Run Now

Select this option if the user wants to run the report immediately.

Schedule

Select this option if the user wants to run this report on a scheduled basis. Once this option is selected the following choices are:

  • Run Once
    • Start Date and Start Time to run the report.
  • Repeat
    • Select Daily, Weekly, Monthly or Yearly for the appropriate re-occurrence for running the report.
      • If Daily is selected, check the Weekdays only box to limit to weekdays.
      • If Weekly is selected, check which weekday to run the report.
      • If Monthly is selected, check the month to run the report.
      • If Yearly is selected, select either the date or the weekday to run the report.

Date Range – Select No End Date or a date that the re-occurrence should end.

Data Element/Column Header

Description

Incident ID

The PowerSchool incident ID.

TX Discipline Incident Number

The Texas Disciplinary Incident Number assigned to the record; required for TSDS PEIMS reporting (E1016).

School Number

The school number associated with the record.

Incident Date

The date of the incident.

Report Period

The Texas Reporting Period code (1-6) associated with the Incident Date; this value is calculated within the report.

TitleThe title of the incident as entered in the Incident Management record.
Description

The description of the incident as entered in the Incident Management record.

Location

The value from the Location dropdown list.

Location State Code

The state code mapping for the Location (01-05).

Student Name

The student’s name, output as Last First Middle.

Local IDThe student's local student ID number.

Unique ID

The student’s Texas Unique ID; required for TSDS PEIMS reporting (E1523).

SPED Indicator

The student’s Special Education Indicator Code based on the Special Education special program record in effect on the Incident Date.

BehaviorThe Behavior code assigned to the student.

Behavior State Code

The state code mapping for the Behavior, which corresponds to the TEA Disciplinary Action Reason Code (E1006) reported in the Discipline Interchange. In order to be included in the Discipline Interchange, the incident must have a state-reportable Behavior Code and a state-reportable Action Code.

Test Behavior Attached to Offender

Displays "Pass" if the Behavior Code is correctly associated to the student; otherwise "Missing." 

Test Behavior State Reportable

Displays "Pass" if the Behavior Code is mapped to a valid state code; otherwise "Missing/Invalid." 

Test Primary Behavior

Displays "Pass" when applicable; otherwise "Missing." 

  • This field is not required for any student who has only one behavior in the incident; the value automatically displays as "Pass.".
  • If a student has more than one behavior and the Primary Flag is set for one of them, "Pass" is displayed for that behavior and "N/A" is displayed for the other(s).
  • If a student has more than one behavior and the Primary Flag is not set, "Missing" is displayed.

Action

The Action code assigned to the student.

Action State Code

The state code mapping for the Action, which corresponds to the TEA Disciplinary Action Code (E1005) reported in the Discipline Interchange.

Test Action State Reportable

Displays "Pass" if the Action Code is mapped to a valid state code; otherwise "Missing/Invalid." In order to be included in the Discipline Interchange, the incident must have a state-reportable Behavior Code and a state-reportable Action Code.

Action Begin Date

The Begin Date entered in the Action Date Range in the incident, which corresponds to the Date of Disciplinary Action (E1036) reported in the Discipline Interchange.

Test Action DateDisplays "Pass" if the Action Begin Date is populated in the incident; otherwise "Missing."
Action End DateThe End Date entered in the Action Date Range in the incident.
Assigned Duration

The official length in days of a student’s disciplinary assignment as entered in the Assigned Duration field in the incident. This is reported as the Official Length of Disciplinary Assignment (E1007) in the Discipline Interchange.

Test Assigned DurationDisplays "Pass" if the value is populated in the incident; otherwise "Missing."

Actual Duration

The actual length in days of a student’s disciplinary assignment as entered in the Action Duration field in the incident. This is reported as the Actual Length of Disciplinary Assignment (E1008) in the Discipline Interchange.

Test Actual DurationDisplays "Pass" if the value is populated in the incident; otherwise "Missing."

Duration Code

The reason for the difference, if any, between the official and actual lengths of a student’s disciplinary assignment as entered in the Duration Code field in the incident. This is reported as the Disciplinary Length Difference Reason Code (E1009) in the Discipline Interchange

Test Duration CodeDisplays "Pass" if the value is populated in the incident; otherwise "Missing."

Campus of Responsibility

The school number of the instructional campus on which the student was enrolled when the discipline incident and action occurred; for a continuation disciplinary assignment, this is the campus that the student would attend under all normal circumstances if not under a disciplinary assignment. This is entered in the Campus of Responsibility Action Attribute field.

Campus of Assignment

The school number of the campus in which the student was placed for disciplinary reasons as entered in the Campus of Assignment Action Attribute field; required for TSDS PEIMS reporting when the discipline results in a placement (E1003).
