Physical Restraint and Seclusion File

Introduction

This report includes a record for each school. The report includes counts of incidents at each school for various attributes related to restraint and seclusion discipline actions.

How to Use: To successfully run the Physical Restraint and Seclusion report the following steps must be completed:

  • Set up the Incident Management Codes:

    • Go to Start Page > District Setup > Incident Management Setup > Code & Subcode Setup

    • Set up the following Action Codes:

      • State Discipline Action > Restraint

        • State Detail Report Code: 6

      • State Discipline Action > Seclusion in Unlocked Room

        • State Detail Report Code: 7

      • State Discipline Action > Seclusion in Locked Room

        • State Detail Report Code: 8

    • Set up the following Action Attribute Codes:

      • Staff Status

        • Data Type: Select

        • State Aggregate Report Code: SS

        • Staff Status > Untrained

          • State Detail Report Code: US

        • Staff Status > Trained

          • State Detail Report Code: TS

      • Action Resulted in Injury

        • Date Type: Yes/No

        • State Aggregate Report Code: INJ

      • Action Resulted in Death

        • Date Type: Yes/No

        • State Aggregate Report Code: DTH

  • Fill in the number of seclusion rooms, description of seclusion rooms, and description of steps taken to reduce incidents per school. These data entry fields are available on the Start Page > District Setup > Schools/School Info > Edit School page. Note that while the text fields and the extract both allow up to 1000 characters, line feeds and commas will be removed when the data is reported.

Note: PowerSchool version 7.10.2 or higher is required in order to create the Yes/No data type codes.

The report includes the following features:

  • The extract output is a comma delimited .csv file.

  • The report can be run for all schools or a selection of schools, for a selection of students or for all students.

Selection Criteria

Selection criteria determine which database records are used in the report. Refer to selection criteria for analysis when the report does not return the correct records.

The report selects records from the Incident Management tables based on the following criteria:

  • The school associated with the incident must not be excluded from state reporting.

  • The student must not be excluded from state reporting.

  • The incident date must occur between the dates provided at run time.

  • The incident must include an action of Restraint, Seclusion in an Unlocked Room, or Seclusion in a Locked Room.

Report Input

To generate the Physical Restraint and Seclusion report, do the following:

Description

Select Schools

Choose one of the following:

  • Current School Only – Includes all student records associated with the current school that meet selection criteria. To change the current school, click the School link at the top of the page and choose the appropriate school from the list.

  • All Schools – Includes all student records on the PowerSchool server that meet selection criteria.

Note: The All Schools option is only available when running the report from the District Office.

Students to Include

Indicate which students to include in the report by selecting one of the following options:

  • The selected [number] number of students only – Run the report for students in the current selection.

  • All Students (Default) – Run the report for all students in the current school or district that meet the selection criteria.

Note: If running the report for a single student, or group of students, select the students prior to running report.

Report Start Date

Enter the start to restrict students for the report.

Report End Date

Enter the end date to restrict students for the report.

Scheduling

Choose a time to run the report from following options:

  • Run Now (default)

  • Schedule
