(GD) Student Discipline Record – New!

Introduction

The (GD) Student Discipline Record - New! report provides required Federal and State information regarding reportable discipline actions.

Selection Criteria

Selection criteria determine which database records are used for the (GD) Student Discipline Record - New!:

School Selection

The report selects which school records to use based on the following criteria:

  • The school must be associated with a student included in the report.

Student Selection

The report selects which student records to use based on the following criteria:

  • The student must be enrolled during the report start and end dates.
  • The student is not excluded from state reporting (State/Province-OH>Home Tab> State_ExcludeEnrollment).

Log Selection

The report selects which Log Entries to include based on the following criteria:

  • The Action Date must be within the report start and end dates.

The report input is done in Incident Management with the codes and subcodes used in IM.

Description

Schools for which to run?*

Choose one of the following:

  • Select Multiple Schools – Includes all student records associated with the school(s) that meet the selection criteria
  • All Schools (default) – Includes all student records on the PowerSchool server that meet the selection criteria.

Current Selection Students*

Indicate which students to include in the report by selecting one of the following options:

  • The Selected [number] Students Only – Run the report for students in the current selection.
  • All Students – Run the report for all students in the current school or district that meet the selection criteria.

Note: If running the report for a single student, or group of students, select the students prior to running report.

Report Start Date

Enter the start date for reporting period.

Format: [M]M/[D]D/[YY]YY

Report End Date

Enter the end date for the reporting period.

Format: [M]M/[D]D/[YY]YY

Reporting Period

This is defaulted to S.

Fiscal Year

Enter the fiscal year for the report.

Scheduling

Run Now

Select this option to execute the report in the order it is received in the report queue.

Schedule (available Version 7.1.1.)

Enter the start date and start time for the report.

  • Select Run Once and Submit to run the report at the specified date and time.
  • Select Repeat to set future dates (Daily, Weekly, Monthly, Yearly) and Submit.

This field contains the school building number. This field value is extracted from the Edit School page, Start Page > District Setup > Schools/School Info > Edit School.

EMIS Student ID Number

This field contains the EMIS student ID number assigned by the district to this student. This field value is extracted from the Student Number field on the General Demographics page, Start Page > Student Selection > General Demographics and prepended with the EMIS Prefix ID, if entered, else zeroes are added to create a 9-digit number.

This field contains the date the discipline was administered to the student. This field value is extracted from the Discipline Action Date field on the Log Entry page when the Log Type is Discipline, Start Page > Student Selection > Log Entry.

Discipline Reason (First Reason)

This field contains the primary reason that the student was disciplined. This field value is extracted from the Discipline Reason field on the Log Entry page when the Log Type is Discipline, Start Page > Student Selection > Log Entry.

Discipline Sequence Number

This field contains the order in which the discipline incidents occurred, if the incidents occurred on the same day and the Type of Discipline administered was the same for all incidents. This field value is extracted from the Discipline Sequence Number field on the Log Entry page when the Log Type is Discipline, Start Page > Student Selection > Log Entry. If the field is blank a 1 is the default value for this field.

This field contains the length of the discipline, in Full Time Equivalency (FTE), for each discipline occurrence. This field value is extracted from the Total Discipline Days field on the Log Entry page when the Log Type is Discipline, Start Page > Student Selection > Log Entry.

Discipline Modified

‘Y’ displays if the Chief Administrating Officer modified, in writing, the one-year expulsion requirement for possession of a firearm at school.

‘N’ displays if the one-year expulsion discipline was not modified.

‘* displays if the discipline incident did not result in a one-year expulsion or did not involve a firearm.

This field value is extracted from the Discipline modified on a per exception basis field on the Log Entry page when the Log Type is Discipline, Start Page > Student Selection > Log Entry.

Referred for Alternate Educational Services

‘Y’ displays if the student, for whom the Chief Operating Officer modified as part of a case-by-case basis the one-year expulsion requirement for possession of a firearm, was recommended for alternate educational services.

‘N’ displays if this student was not recommended for alternate educational services.

The default is ‘*’.

This field value is extracted from the Referred for Alternate Program field on the Log Entry page when the Log Type is Discipline, Start Page > Student Selection > Log Entry.
