Introduction
The (GD) Student Discipline Record report provides required state information regarding reportable discipline actions.
Selection Criteria
Selection criteria determine which database records are used for the (GD) Student Discipline Record.
School Selection
The report selects which school records to use based on the following criteria:

The school must be associated with a student included in the report.
Student Selection
The report selects which student records to use based on the following criteria:

The student must be enrolled during the report start and end dates.

The student is not excluded from state reporting (State/Province-OH > Home > State_ExcludeEnrollment or for a previous fiscal year S_OH_STU_SNAP_C.State_ExcludeFromReporting).
Log Selection
When Logs is selected as the data source, the report selects which Log Entries to include based on the following criterion:

The Action Date must be within the report start and end dates.
Incident Management Selection
The report selects which Incident Management records to include based on the following criterion:

The incident date must be within the report start and end dates.
Discipline Reporting Source
Select the source that contains the discipline records to report. The report supports two options:

Logs: Uses discipline log entries and the log-entry fields described in the output table below.

Incident Management: Uses Incident Management records whose incident date falls within the report dates and the Incident Management fields described in the separate output table.

Important: Use the same discipline data source for the entire fiscal year. Do not combine Log entries for part of the fiscal year with Incident Management records for the remainder of the fiscal year.
Report Output
Each of the fields displayed in the output of the report are described below. The report output depends on the selected discipline data source, refer to the output table based on the data source chosen. See Understanding the Report Output Table for a definition of each column in the table. The output is aligned to the Ohio Department of Education parameters for the Student Discipline (GD) Record.
Key reporting considerations for the GD

One record per administered and served discipline type: Report a separate GD record for each Type of Discipline administered to and served by the student for an incident. If discipline spans school years, report the served portion in each applicable year using the same incident date.

Incident ownership and location: The last building in the district where the student was enrolled and the action was administered reports the record. GD120 identifies where the incident occurred and may differ from GD040; use 999999 when the location is a non-EMIS reporting entity.

Date and days: GD060 is the date the behavior/incident occurred. GD090 is the FTE days served for the discipline type in the current school year. Do not report a GD record when no discipline time was served.

Reasons: GD080 supports up to five distinct reasons, with the primary reason first. GD080 First Reason cannot be Not Applicable; use an applicable reason code when discipline is reportable.

Sequence and uniqueness: GD085 must distinguish records for separate incidents occurring on the same date and for multiple discipline types administered for one incident. The unique key is Date of Discipline + Type of Discipline + Sequence Number.
Incident Management Output
When Incident Management is selected as the data source, the report output uses Incident Management records with an incident date within the report start and end dates. The source attributes for the fields below are located in Incident Management. Each field may be calculated from one or more tables; the underlying table relationships are not shown in this table.
