Report Description
The ED205 form is the data collection tool that generates the reports that are included in the statewide federal performance report for Title I activities. The Title I Student field on the Connecticut State Information page is used to identify Title I students, Start Page > Student > State/Province – CT.
The ED205 is comprised of three schedules:
- Schedule 1: Title I district and contact information
- Schedule 2: Title I demographic, staffing and project information
- Schedule 3: Title I Program Evaluation for Nonpublic Schools or Local Neglected Programs that received Title I funds or services
Selection Criteria
Selection criteria determine which database records are used in the report. Refer to selection criteria for analysis when the report does not return the correct records.
Student Selection
The report selects records from the [Students] table based on the following criteria:
- The student must have the Title I Student field, [S_CT_STU_Demographics_X]TitleIStudent, set to Y.
- The student must not be excluded from state reporting.
- The student must have an enrollment within the selected term.

Field | Description |
Use | If run at the district level, choose one of the following from the pop-up menu:
If run at the school level, the report includes all records associated with the current school that meet the selection criteria. To change the current school, click School at the top of the page and choose the appropriate school from the list. |
Students to Include | Indicate which students to include in the report by selecting one of the following options:
Note: If running the report for a single student, or group of students, select the students prior to runningreport. |
Processing Options | Choose a time to run the report from the pop-up menu:

Specific Date/Time | Enter a date using the format mm/dd/yyyyor mm-dd-yyyyin order to run the report on a specific day. The incorrect format displays an alert and the date field is submitted as a blank entry. Use the pop-up menus to schedule the report to be run at a specific hour and minute. Example: 7/29/2008 @ 10 AM : 05. |
Name | Enter the name of the contact that you want to print on the report. |
Street Address | Enter the contact’s street address. |
Town | Enter the town where the contact’s street address is located. |
State | Enter the state where the contact’s street address is located. |
Zip Code | Enter the zip code for the contact. |
Telephone Number | Enter the contact’s telephone number. |
Fax Number | Enter the contact’s fax number. |
Enter the contact’s email address. | |
District Name | If the contact is responsible for multiple districts, enter the name of each district. You can enter up to six districts. Only use this field if the contact is responsible for a regional district. |
District Code | For each district name entered, type the district code. You can enter up to six district codes. Only use this field if the contact is responsible for a regional district. |
Select a Term to Run | Choose a school term for the report. |
Schedule 1 – Title I District and Contact Information
