Notification Letter Required Immunization for Admission to Wisconsin Schools

This will create a Notification Letter to parents/guardians of students with immunization status of No Record, In Process, or Behind Schedule. The letter is sent on the 15th and 25th school days. Run the Update Student Compliance function before running this report.

This replaces the Legal Notice starting with the 2025-2026 School Year.

Select the Current School Only.

This is the only option available at the School level.

School Official (Title)

Enter the name of the school official.

Date Sent

Enter the date of the letter.

Contact - Person to contact if Parent/Guardian has questions.

Enter the name of the contact person.

This will produce a notification letter per student of the following PDF: template-notification-letter-required-immunizations-school English.pdf
