Programs Fact Services Extract

Introduction

This template tracks services received by students for the Community Schools Program (CSP), both Community Schools Act Programs as well as Title I funded Community Schools. It also tracks services received by students for the Extended Learning Time Program (ELTP).

The extract reports data based on which school the student was enrolled in on the date of the reporting period selected when the report is run.

Selection Criteria

Selection criteria determine which records are reported.

Program Fact Services Selection

The report selects Program Fact records from the [S_NM_ProgramServFact_C] table based on the following criteria:

The Program Fact Services record start date must occur on or before the date associated with the selected reporting period.
The Program Fact Services record end date must occur after the date associated with the selected reporting period.

Student Selection

The report selects student records from the [Students] or [ReEnrollments] table based on the following criteria:

The student must not be excluded from state reporting.
The student must be enrolled in a school included in the report.
The student is HSDA-flagged.

Field	Description
Select Schools	Choose one of the following: Current School Only – Includes all student records associated with the current school that meets selection criteria. To change the current school, click the School link at the top of the page and choose the appropriate school from the list. All Schools – Includes all student records on the PowerSchool server that meet selection criteria. The All Schools option is only available when running the report from the District Office.
Select Students	Choose one of the following: The Selected [x] Students Only – Run the report for students in the current selection. All Students – Run the report for all students in the current school or district.
Reporting Period	Choose a reporting period from the pop-up menu: 40D 80D 120D EOY Summer

Report Output

Each of the fields displayed in the output of the report is described below. See Understanding the Report Output Table for a definition of each column in the table.

If a student participated in all three programs during the reporting period, a minimum of 3 records would be submitted per student indicating the type of service received. In addition, a student may have received multiple services for a single program.
