Accommodation
All students, including students with disabilities and English Learners, are required to participate in statewide assessments and have their assessment results be part of the state’s accountability systems, with narrow exceptions for certain students with disabilities who have been identified by the Individualized Education Program team to take their state’s alternate assessment. This form captures the accommodations provided to the student.
