How to Define the Incident Description
Use the steps below to populate the Incident Description portion of an incident. See the Appendix for code values.
- Log into the school or district associated with the incident.
- Navigate to Start Page > Special Functions > Incident Management > Create New Incident.
- If you are in District mode, choose a school from the School pop-up menu. This option is only available if the incident is created at the district. This option is not available in School mode.
- Choose WY State Reporting from the Incident Type pop-up menu.
- Enter the Incident Date and Time based on when the incident occurred.
- Choose the appropriate value from the Time Frame pop-up menu.
- Enter a Title for the incident (optional).
- Enter a Description for the incident (optional, but encouraged).
- Choose the appropriate value from the Location pop-up menu.
- Enter a Location Description if applicable.
- Enter the first and last name of the person responsible for entering the incident in the Prepared by field (optional).
- Enter the Financial Impact if the incident involved vandalism. Enter as a whole number format.
- Click Submit Incident to save the changes.
