How to Define the Incident Description

Use the steps below to populate the Incident Description portion of an incident. See the Appendix for code values.

  1. Log into the school or district associated with the incident.
  2. Navigate to Start Page > Special Functions > Incident Management > Create New Incident.
  3. If you are in District mode, choose a school from the School pop-up menu. This option is only available if the incident is created at the district. This option is not available in School mode.
  4. Choose WY State Reporting from the Incident Type pop-up menu.
  5. Enter the Incident Date and Time based on when the incident occurred.
  6. Choose the appropriate value from the Time Frame pop-up menu.
  7. Enter a Title for the incident (optional).
  8. Enter a Description for the incident (optional, but encouraged).
  9. Choose the appropriate value from the Location pop-up menu.
  10. Enter a Location Description if applicable.
  11. Enter the first and last name of the person responsible for entering the incident in the Prepared by field (optional).
  12. Enter the Financial Impact if the incident involved vandalism. Enter as a whole number format.
  13. Click Submit Incident to save the changes.
