Restraint and Seclusion Incidents: Data Collection Updates

The Student Incident Management data entry screen is updated to have four new Action Attributes added under Injury or Death Flag (and above the Comment field).

Action Resulted in Injury to Student (Yes, No, Blank)
Action Resulted in Injury to Personnel (Yes, No, Blank)
Action Resulted in Death of Student (Yes, No, Blank)
Action Resulted in Death of Personnel (Yes, No, Blank)

The Student Incident Management data entry screen is updated to have two new options for the Seclusion and Restraint Action Attributes Steps Taken fields (all three fields):

260 - Planned Ignoring
270 - Increased Home/School Communication
