Service Code

Select the relevant service code. 

A numeric value will be saved but the report will return the applicable text value.

For the EELTO Program Code, only the new code (34) ASPROGRAMS is available and is selected as the service code by default. All other service codes are disabled. 

For the CSP Program Code, the following options under the Health and Wellness category are disabled:

• Counseling Services 

• Dental Services

• Home Visits

• Social Work Services

• Vision and Hearing Services

• Wrap-Around Services

Enter the name of the entity providing the service.

Note: If the Community Schools Act school is providing the service, use the word School (field will default to this).

If a Community Service Provider is providing the service, enter the name of the entity, not the individual’s name.
