Update to Values for Text-To-Speech Fields on NJ SLA tabs

When navigating to the student State/Province-NJ > NJSLA tab, wording "2015-2016 Field Value: Not Needed" is removed, and the description for value (02) in the Text to Speech (Math) field displays as (02) Text-to-Speech English; Math and Science Text and Graphics. 

When navigating to the State/Province-NJ > NJSLA-S tab, value (01) for Text-to-Speech is inactive, the description for value (02) displays as Text-to-Speech English for Math and Science Text and Graphics, and a new value is added for (04) Text-to-Speech Spanish; Math and Science Text and Graphics.

NJ SMART Career and Technical Data Extract Report: Update to include Preferred or Legal Name Option

NJ SMART Career and Technical Education Extract: version 1.8.

To enable the ability to extract either a student's preferred name or legal name, the report page is updated with a new report parameter named Student Name to Print, with the options to select Preferred or Legal.

NJ SMART SID Student Tab: Update field label description for Determined to be Ineligible

The field label description for Determined Eligible is updated as follows:
This field is used to calculate the Special Education value reported in the SID report using state validation rules. A value of 00 for Special Education requires that this field be set to Yes.

School Health History and Appraisal Updates

NJ Student Health History and Appraisal Report: Version 1.1

The following updates are made:

• The report title is changed from NJ School Health History and Appraisal Report to NJ Student Health History and Appraisal Report.

• The report includes all students in the selection who are not excluded from state reporting.

• The report layout is updated to include sections for Lead Screening, Titer information, Diagnosis and Chronic Conditions, Vision with and without correction, and Vision Muscle Balance.

• The physical examination section now has additional lines.

• The Chest X-Ray section now has space for dates and check boxes for normal/abnormal results. The date of the chest x-ray displays in the date column (Chest X-Ray Film Date) and, based on the the codes and names in the Chest X-Ray Impressions field, 'x' displays in the appropriate column for the results.

Reminder: The report only prints stored corresponding student data for each section.
