NJ SSDS Other Incident Leading to Removal Report

Introduction

This report is used to report any State Reportable Incident leading to the removal of a student where the behavior has a State Aggregate Code of OTHSSDS^.  It should not be used to report any of the following incident behaviors as these are reported on the SSDS Incident Report Form:

Arson

Substance Possession

Assault

Substance Sale/Distribution

Computer Trespass

Theft

Damage to Property

Threat - Simple

HIB Alleged

Threat – Criminal – Aggravated Assault

HIB Confirmed

Threat – Criminal – Arson

Kidnapping

Threat – Criminal – Homicide

Robbery/Extortion

Threat – Criminal – Kidnapping

Sexual Assault

Threat – Criminal – Sexual Assault

Sexual Contact

Trespass

Substance Use Confirmed

Weapon Possession

Substance Use Suspected, Refused Exam

Weapon Sales/Distribution

Removal

Removal is defined as:

  • ISS

  • OSS

  • Removal to Another School

  • Removal to alternative education program

  • Expulsion

  • Unilateral Removal

  • Removal by Hearing Officer

Note: Removal to Other is not included

Incidents Included

  • Within the user-specified date range, any incident that involves removal of the Offender from the school where the incident occurred is included in the report output.

Offenders

  • If there is more than one Offender for an Incident, each Offender's actions and details are reported on a separate page.

  • "Unknown" Offenders are not reported.

  • The Offender count on page 1 includes all Offenders, including Staff and Non-students, minus "Unknown" Offenders.

Selection Criteria

Selection criteria determine which database records are used in the report. Refer to selection criteria for analysis of report results when the report does not return the correct records.

Student Selection

  • The date of the incident must be between the Incident Start Date and the Incident End Date.

  • The student must be from the school where the incident occurred (Participant Attribute = GENERAL EDUCATION STUDENT^ or STUDENT WITH DISABILITIES^).

Description

Select Schools

Choose one of the following:

  • Current School Only: Includes all student records associated with the current school meets the selection criteria. To change the current school, click the School link at the top of the page and choose the appropriate school from the list.

  • All Schools: Includes all student records on the PowerSchool server that meet the selection criteria.

Notes:

  • Incident School, not Participant School, is used to select the Incidents to report (unless run from the District Office and the All Schools option is selected).

  • The All Schools option is only available when running the report from the District Office.

Select School Year

The default option is the current year. If the incident occurred in a previous school year, choose that year.

Incident Start Date

Enter the start date of the date range of incidents you want to report. Only incidents that occurred on or after this date will be reported.

Incident End Date

Enter the end date of the date range of incidents you want to report. Only incidents that occurred on or before this date will be reported.

Indicates whether the student was assigned one of the following forms of removal, other than suspension:

  • None

  • Removal to another school

  • Removal to alternative education program

  • Expulsion

  • Unilateral removal

    • Days. The number of days the student was unilaterally removed.

  • Removal by hearing officer

    • Days. The number of days the student was removed by a hearing officer.
