This table is a stand alone table that stores information about the student discipline actions.

| HAS_FATALS | 13.6.0 | CHAR | Indicates that there are fatal validation errors related to this action record. |
| INCIDENT_ID | 13.6.0 | NUMBER(10,0) | This incident id of the incident. |
| OFFENDER_ID | 13.6.0 | NUMBER(10,0) | The internal id number from students or staff for the offender on the incident. |
| OFFENDER_TYPE | 13.6.0 | VARCHAR2(20) | Indicates if the offender is a student, staff or someone not in PowerSchool. |
| RPT_RUN_ID | 13.6.0 | NUMBER(,) | Indicates the unique report run ID for this run. This links to the S_NC_RPT_RUN table to identify the LEA, School, Report Type, Instance ID, and Year ID. |
| VICTIM_ID | 13.6.0 | NUMBER(10,0) | The internal id number from students or staff for the victim on the incident. |
| VICTIM_TYPE | 13.6.0 | VARCHAR2(20) | Indicates if the offender is a student, staff or someone not in PowerSchool. |
