PSSR-312613	896 Immunization Certificate Form Update	896 Immunization Certificate Form: Version 1.2 To show a student is in compliance when they have had Varicella (Chickenpox) the following updates are made: The Exemption Type with the State Report Code of ‘varicella_disease’ will report in the Mo/Yr format in the Varicella Disease column of the 896 Immunization Certification Form. If a student has an exemption added to Varicella, and that exemption has a Stare Report Code of ‘varicella_disease’, the student is considered to be in compliance. For districts without any Exemption Types; a new ‘History of Varicella (Chickenpox)’ Exemption Type is added with a State Report Code of ‘varicella_disease’. Note: Districts using Health for immunizations need to create or edit an Exemption Type with the State Report Code of ‘varicella_disease’ to report students who have had Varicella on the 896 Certificate and to indicate the student is in compliance.
