KS: Updates to the CourseWalk Report (KCCMS)

Following changes are made to the CourseWalk Extract report in version 1.9:

The extract would only contain 19 columns (C1-C19).
The following columns are updated in the template:
> C15 is for 'KSDE Use Only', with a blank value being displayed in the report.
> C16 will contain 'State Subject Area Code'
> C17 will contain 'State Course Identifier'
> C18 will contain 'Effective Date'
> C19 will contain 'Expiration Date'
