Create a Discipline Incident

Introduction

The following data elements are required for discipline incidents. You create incidents at the school or district level, rather than at the student level. If an incident occurs on school grounds, PowerSchool recommends creating the incident while logged into that school. If the incident occurs within the district, but is not related to a particular school, PowerSchool recommends creating the incident at the district level.

To create incidents, you must have permissions to access the Alabama State Reporting incident type. Refer to Group Security Permissions for more information.

Create an Incident

Navigation: Start Page > Special Functions > Incident Management > Create New Incident (for PowerSchool SIS 20.4.0 and higher, choose Create Detailed Incident)

  1. For Incident Type, choose Alabama State Reporting.

  2. Enter the Incident Date and Time.

  3. Choose the Time Frame in which the incident occurred.

  4. Enter the incident's Title and Description.

  5. Choose the Location at which the incident occurred

  6. Add further information (see below) or click Submit Incident.

Incident Builder - Add Participants

Use the steps below to add the participants - students and staff - who were involved in the incident. You can add student and staff who are in your PowerSchool system, as well as those who are not.

Add Student and Staff Participants

Use these steps to add the student or staff member who was involved in the incident and who is in the PowerSchool system.

  1. Under Incident Builder, click the plus sign (+) next to Participants.

  2. Enter the search criteria and click Search.

  3. Click the participant and click Add.

  4. Click the plus sign (+) next to Attributes and choose the offense or attribute.

  5. Choose a role.

    • There must be at least one student offender.

    • If the participant has more than one role, click the plus sign (+) to add another role.

  6. Click Add Participant Attributes.

  7. Add more participants, add further information (see below), or click Submit Incident.

Add Other Participants

Use these steps to add participants who are not in the PowerSchool system.

  1. Under Incident Builder, click the plus sign (+) next to Participants.

  2. Click Create Other.

  3. Enter the participant's first, middle, and last name.

  4. Enter the participant's position and age, and choose their gender.

    • This information is required for both Offenders and Victims, but is not required for Witnesses or Reporters.

  5. Click Add.

  6. Choose a role.

    • There must be at least one Offender per incident.

  7. Under Attributes, click the plus sign (+) and choose a Participant Attribute code.

  8. Click Add Participant Attributes.

  9. Add further information (see below) or click Submit Incident.

Incident Builder - Add Incident Elements

Use the steps below to add behaviors, actions, objects, and attributes to the incident.

Add an Action

An action is the means taken to address the incident.

  1. Click the plus sign (+) next to Incident Elements and choose Add Action.

  2. Choose the applicable Action Code, and the action subcode if applicable.

  3. Enter the action's date range. Select the Begin Date and Expected End Date.

    • The date reported is the next in-session day after the action end date. 

  4. Add further details about the action taken, if required.

  5. If the action is complete, enter the Actual End Date.

  6. Choose a Duration Code, enter the Assigned Duration and the Actual Duration, and add Duration Notes, if required.

  7. If the action taken has changed, enter the reason, and then choose an Action Change Code (and subcode, if applicable)

  8. Under Action Attributes choose the action's attributes, if applicable.

  9. Click Add Action.

  10. Drag the action to the participant. The action appears under the name of the participant and is no longer listed under Incident Elements.

Add an Object

An object is an item used during the incident; for example, a weapon or personal property.

  1. Click the plus sign (+) next to Incident Elements and choose Add Object.

  2. Choose the object used by the offender and choose a subcode, if applicable.

    • Define each object only once; it is not necessary to add the object to both the student and the incident.

  3. Enter a description of the object and the quantity involved.

  4. Click Add Object.

  5. Drag the object to the applicable participant. The object appears under the name of the participant and remains listed under Incident Elements.

Add a Behavior

Add a primary behavior and other behaviors. Only the primary behavior is reported.

  1. Click the plus sign (+) next to Incident Elements and choose Add Behavior.

  2. Choose the applicable Behavior Code and then choose the appropriate behavior subcode, if applicable.

  3. Select Primary Behavior for the most serious behavior (even if there is only one behavior in the incident).

  4. If the behavior has not been proven, select Allegation.

  5. Click Add Behavior.

  6. Drag the behavior to the participant. The behavior appears under the name of the participant and also remains listed under Incident Elements

Add an Attribute

  1. Click the plus sign (+) next to Incident Elements and choose Add Attribute.

  2. Select one or more attributes.

  3. Click Add Attributes.

  4. Click Submit Incident.
